E-Maintenance Request Form
All maintenance requests MUST be in writing.  Please fill out the e-form below and your maintenance request will
automatically be e-mailed to us.  If you would like to contact us by phone, in addition, to this E-Maintenance Request, please call
512-248-0022.  After you are finished filling out this form, press the "Submit" button to submit your information.  We will schedule your
maintenance the next business day.  If you make a mistake before you send the maintenance request, simply use the reset button to
clear all the form fields and start again.  Our maintenance system will send you a confirmation email after you click the submit button.

So that we can provide you with a quick response, please be as specific as possible when describing the problem.  Requests that
are not specific (i.e. garage-door not working) will delay our response while we attempt to contact you for clarification.

As per your lease agreement, if the work requested is found to be caused by the resident or is the resident's responsibility, you will
be billed for the charges.  You will be responsible for the payment of any service call charged for: a missed appointment, not
providing access to your unit when requested, leaving a key-less bolting device engaged or not following other instructions as you
agreed to resulting in the vendor not gaining entry to the property.  

The licensed vendors we use are NOT employees of Mayfield-DeWolf & Associates.  If a vendor fails to keep an appointment, does
not complete a repair, does not follow up with another appointment (if needed to complete the job) or if you feel you have received
unsatisfactory service or work, please let us know asap.  We nor any of our vendors will enter your home at anytime if someone under
18 years of age is at home without adult supervision.  No exceptions.
Maintenance Emergencies
If your maintenance request is an emergency (an immediate threat to life or property) please contact us during normal business hours
at 512-248-0022 and please identify that you have a maintenance emergency.  If your emergency is after hours, please call the same
phone number, but leave a message and a staff member will return your call as soon as possible.
